    Do you accept private insurance?

    Yes, we accept some private insurances. Please call to confirm if we
    accept your plan.

    Do you accept Medicaid and Medicare?

    Yes, we accept most Medicaid and Medicare plans. Please call
    to confirm if we accept your plan.

    How do I make an appointment?

    You can conveniently book your appointment on our website or call
    us to schedule.

    Do you accept walk-ins?

    Yes, we accept walk-in patients. Walk-ins are evaluated and seen in order of
    urgency of illness or health issue.

    What should I bring with me on my first visit?

    Please bring identification – this may be a driver’s license, a state issued photo ID, or a birth certificate if you do not have a driver’s license to verify your
    date of birth. Please bring your insurance card or proof of insurance. If you are uninsured, we will require proof of all household income and/or a notarized letter of support.

    How much do you charge for your services if I don’t have insurance?

    The cost of our services is determined by a sliding fee scale based on total household income, the number of persons living at your residence, and in accordance with the current Federal Poverty Guidelines. We offer up to a 55% discount on services and offer a payment plan if not able to pay for services in full.

    What proof of income do you accept?

    Pay stub, bank statement, unemployment letter with income, and/or SSI letter with income. If you have no income, a notarized letter of support.

    New Patients- Please arrive 30 minutes before the appointment to get checked in
    (If you complete the office forms prior to arriving, this will expedite your check in
    process).

    Existing Patients- Please arrive 15 minutes before your appointment to verify
    information.

    Missed appointments will be subject to a fee of $15 for medical appointments and
    $10 for labs appointments if not cancelled at least 24 hours in advance.
